Midwestern Pet Foods, Inc., has been issued a formal warning by the FDA after inspections of their manufacturing plants revealed several violations.

The FDA claims the conditions at the facilities were likely responsible for the illness and death of hundreds of dogs and cats.

The FDA found extremely high levels of aflatoxin in food samples from the manufacturing facility.
